The Coven 13: Nomination Guidelines & Terms

Who can be nominated?

• Women aged 18 or older who reside in Ontario.

• Women who could benefit from additional support navigating their hormonal or general health concerns, particularly those facing barriers to accessing care.

• You may nominate yourself or someone you know.

• No purchase or payment is necessary.

• Nominations are open until October 31, 2026, at 11:59 p.m. ET.

• Employees, contractors and immediate family members of the Coven Women's Health team are not eligible.

How does it work?

• Complete the nomination form, sharing a brief explanation of why you or the woman you're nominating could benefit from additional support.

• Please do not include sensitive medical information or personal health details about someone else.

• Coven Women's Health will review nominations and select 13 recipients based on demonstrated need, potential barriers to accessing care and the anticipated benefit of a Healthcare Expert Consultation & Navigation session.

• Recipients will be contacted privately. If you've nominated someone else, we will confirm her interest and consent before arranging care.

• Selected recipients must confirm their interest and eligibility within five business days of notification. If a recipient declines, cannot be contacted or is ineligible, Coven may select an alternate recipient.

• Multiple nominations of the same person do not increase the likelihood of selection.

• Meeting eligibility requirements does not guarantee a complimentary session.

What is included?

Each selected recipient will receive one complimentary 30-minute Healthcare Expert Consultation & Navigation session with a Coven Registered Nurse Health Coach, valued at $99.

The session provides education, guidance and support navigating healthcare options and identifying potential next steps. It does not include medical diagnosis, prescriptions or ongoing treatment and is not a substitute for primary care, emergency care or a medical assessment.

Sessions are delivered virtually to eligible Ontario residents.

Coven will make reasonable efforts to schedule all 13 complimentary sessions by December 31, 2026, subject to clinician and recipient availability. Appointments must be booked through Coven's designated booking process.

Sessions have no cash value, are non-transferable and cannot be exchanged for other services.

Privacy & Consent

All nominations will be reviewed confidentially. Personal information will be used to administer this initiative, assess eligibility and contact potential recipients, in accordance with Coven's Privacy Policy.

Recipients will not be identified or featured publicly without their explicit consent.

By submitting a nomination, you confirm that the information provided is accurate to the best of your knowledge and that you agree to these guidelines.

Participation in the nomination process does not establish a patient-provider relationship. Any clinical relationship begins only upon appropriate intake, consent and acceptance into care.

Additional Conditions

Coven reserves the right to verify eligibility, decline incomplete, duplicate or fraudulent submissions, and make reasonable changes to the initiative where necessary, subject to applicable law.

The Coven 13 is a community care initiative, not a random prize draw. Recipients are selected based on eligibility, demonstrated need and barriers to accessing care.
